Diamond Dental Studio of Redmond – Membership Plan Guidelines

General Guidelines

• This is an annual membership plan and cannot be carried over beyond the enrollment period.
• We recommend scheduling your next appointment in advance to ensure continuity of care.
• If your plan lapses, a $50 reinstatement fee will apply.
• If you need to reschedule or cancel an appointment, please provide at least 48 hours' notice.

Membership Plan Exclusions & Limitations

This is a membership plan, not a dental insurance plan. It cannot be used for:

• Any insurance plans (this plan cannot be combined with insurance).
• Patients with insurance plans where Diamond Dental Studio of Redmond is in-network.
• Third-party financial programs, such as CareCredit.
• Referrals to specialists.
• Dental care covered under automobile or medical insurance.
• Services for injuries covered under worker’s compensation.
• Treatment at any other dental office or hospital.

Additional Terms

• Memberships are non-transferable.
• There are no refunds, even if benefits are not used during the enrollment period.
• Discounts are subject to change without notice.
• This plan can be used by individuals who do not have insurance or are out of network but cannot be used in conjunction with out-of-network insurance plans.
